‘04.6월 타병원 ER 방문 →PTE Dx 후에 t-PA & heparin Tx 중 환자 원하여 본원으로 transfer.

but, aPTT 31 sec aCT :110
일단,
3,000 U/IV bolus 후 800U/hr로 start.
F/U한 aPTT 32 sec → heparin infusion 1,000u/hr →
1,200U/hr로 증량

현재로선 congenital AT Ⅲ deficiency 가능성이 높다고 생각됩니다.
1. AT Ⅲ supply to AT Ⅲ level up to 30-40%이상
2. continue Wafarin 5mg/day, 3. family study for AT Ⅲ level이필요하겠습니다.

Heparin 1,500u/hr에도 불구하고 prolongation되지않아 prot.C.S, ATⅢ 등 lab check

warfarin 사용하면서 INR 2～2.5 까지 유지 예정
1. Multifocal thrombosis from infrahepatic IVC to the both common iliac veinlevel.
2. Well-demarcated 2.5 cm sized cystic lesion in the probable left ovaryorigin.
-- R/O Functional cyst.
3. Wedge shaped low density lesion in spleen. -- Previous infarctionsequalae.
Otherwise, unremarkable.
1. Slightly decreased extent of pulmonary thromboembolism in the abovementioned lesion
2. Radiolucent area due to perfusion defect, left lung and right lower lobe.
3. Multifocal pulmonary infarction in both lung.
4. Thrombosis, infrahepatic IVC to left iliac vein.
Suspicious thrombosis in the left popliteal and deep femoral vein.
5. Right ventricular enlargement and pericardial effusion.
R/O Pulmonary hypertension.
6.19 pul. thromboembolism의 원인은 Lt.lower ext.의 DVT에 의할 가능성 있으며congenital ATⅢ deficiency도 가능성 있음.
현재 AT-Ⅲ supply하면서 Heparinization 중 Warfarin 함께 사용하면서 목표 INR 도달될 때까지 유지 예정임.

■ 의사 소견서
상기 환자는 massive pulmonary embolism으로입원하신 분으로 헤파린 치료가 필요하나, 하루 35,000 IU 이상의헤파린 infusion에도 불구하고 aPTT 증가가 없었으며안티트롬빈 Ⅲ level(정상치 : 80-120%)이 26%로 매우 떨어져 있어 안티트롬빈 Ⅲ 주사를 헤파린과 함께 투여하였고 그 이후 aPTT 및 안티트롬빈 level이 적절한 치료농도로 유지되었음.

■ 심의내용
- 안티트롬빈Ⅲ주는 “선천성 antithrombin 결핍에기인하는 혈전 색전 합병증의 예방 및 치료와 후천성 antithrombin Ⅲ 결핍의 예방 및 치료”에식약청장 효능효과를 받은 약제이며, 인정기준(고시 제2001-28호, ‘01.6.8)상 “항트롬빈Ⅲ검사 결과 항트롬빈Ⅲ결핍이 확인되어 유사효능의 타 약제 투여로는 치료효과를 기대할 수없거나 동 약제의 투여가 반드시 필요한 경우” 요양급여를 인정토록 되어 있음.
- 따라서, 동건은PTE 진단하에 tPA 투여 후 Heparin 투여에도불구하고 aPTT가 prolongation 되지 않았고, 안티트롬빈Ⅲ 검사수치결과 36.19%로 감소되어 있는 상태에서 안티트롬빈Ⅲ주를투여한 바, 동 약제의 인정기준에 해당되므로 안티트롬빈Ⅲ주는 인정함.다만, 17일, 18일 안티트롬빈Ⅲ주 투여 후 aPTT가 116초, 83.6초로충분이 prolonged 되었고, 안티트롬빈Ⅲ 결핍이 확인되지않은 상태이므로 19, 20일에 투여된 안티트롬빈Ⅲ주는 인정하지 아니함.
