(경구 : 정제)
성인 : 브롬화수소산덱스트로메토르판으로서 1회 15-30mg 1일 1-4회 경구투여한다.
1일 최대 120mg을 초과하지 않는다.
소아 : 1일 체중 kg당 1mg을 3-4회 분할 투여한다.
연령, 증상에 따라 적절히 증감한다.
(경구 : 서방형 시럽제)
성인 : 브롬화수소산덱스트로메토르판으로서 1회 60mg 1일 2회 경구투여한다.
소아 : 6-12세 1회 30mg, 2-5세 1회 15mg 1일 2회 투여한다.
연령, 증상에 따라 적절히 증감한다.

금기

1) 이 약에 과민증 환자
2) MAO저해제를 투여중이거나 투여를 중지한 지 2주 이내인 환자
3) 천식 환자
4) 이 약은 유당을 함유하고 있으므로, 갈락토오스 불내성(galactose intolerance), Lapp 유당분해효소 결핍증(Lapp lactase deficiency) 또는 포도당-갈락토오스 흡수장애(glucose-galactose malabsorption) 등의 유전적인 문제가 있는 환자에게는 투여하면 안 된다.

1) 간장애 환자
2) 약물과민증 환자
3) 임부, 수유부
4) 만성 기침 환자
5) 소아(특히 2세 이하의 영아)

이상반응

1) 정신신경계 : 졸음, 때때로 두통, 어지러움, 드물게 불쾌, 불면, 흥분, 권태감이 나타날 수 있다.
2) 소화기계 : 때때로 구역, 구토, 식욕부진, 복통, 변비, 드물게 구갈이 나타날 수 있다.
3) 과민증 : 발진 등이 나타날 수 있으므로 이러한 경우에는 투여를 중지한다.
4) 기타 : 드물게 호흡억제가 나타날 수 있다.

[주요 이상반응정보]

일반적 주의

1) 이 약을 투여중인 환자는 졸음을 일으킬 수 있으므로 자동차운전 등 위험이 수반되는 기계조작을 하지 않도록 주의한다.
2) 약물과민증 환자에는 신중히 투여하며 과민증이 나타날 경우에는 투여를 중지하고 항히스타민제 등을 투여한다.
3) 이 약 투여시 음주하지 않는다.

상호작용

MAO저해제(페넬진 등)와 병용투여시 경련, 반사항진, 이상고열, 혼수 등이 나타났다는
보고가 있으므로 병용투여하지 않는다.

소아에 대한 투여

<고령자/노약자>
일반적으로 고령자는 생리기능이 저하되어 있으므로 감량하는 등 주의한다.

적용상의 주의

투여하기 전에 충분히 흔든다.

과량투여 및 처치

과량투여시 혼수, 호흡억제, 경련이 나타날 수 있으며 이러한 경우에는 위세척을 하거
나 호흡억제의 경우에는 나록손을, 경련시에는 벤조디아제핀계 약물을 투여한다.

Mechanism of Action	Dextromethorphan에 대한 Mechanism_Of_Action 정보 Dextromethorphan is an opioid-like drug that binds to and acts as antagonist to the NMDA glutamatergic receptor, it is an agonist to the opioid sigma 1 and sigma 2 receptors, it is also an alpha3/beta4 nicotinic receptor antagonist and targets the serotonin reuptake pump. Dextromethorphan is rapidly absorbed from the gastrointestinal tract, where it enters the bloodstream and crosses the blood-brain barrier. The first-pass through the hepatic portal vein results in some of the drug being metabolized into an active metabolite of dextromethorphan, dextrorphan, the 3-hydroxy derivative of dextromethorphan.
