이 약은 피로아황산나트륨이 함유되어 있어 아나필락시와 같은 알레르기를 일으킬 수 있으며 일부 감수성 환자에서는 생명을 위협할 정도 또는 이보다 약한 천식발작을 일으킬 수 있다. 일반 사람에서의 아황산감수성에 대한 총괄적인 빈도는 알려지지 않았으나 낮은 것으로 보이며 아황산감수성은 비천식환자보다 천식환자에서 빈번한 것으로 나타났다.
이상반응
1) 조절된 임상시험에서 이 약을 8주 동안 사용한 161명의 부작용을 모니터링한 결과, 이 중 102명(63%)이 연구 기간 중 최소한 1개 이상의 치료와 관련된 이상반응을 나타내었다. 가장 흔한 이상반응은 도포 부위에 발생되는 홍반, 낙설, 작열감, 건조감, 소양증이었으며 대부분 경도-중등도에 해당하는 것이었다. 연구자에 의해 이 약과 관련된 것으로 평가된 것으로써, 최소 1% 이상의 환자에서 발생된 이상반응은 (표 1)과 같다. 공개, 장기간 안전성 연구에서, 이 약을 6개월간 지속적으로 사용한 환자들의 이상반응 발현 양상은 8주간 사용한 환자들과 유사하였다.

2) 외용 코르티코스테로이드에서 드물게 보고되는 국소 이상반응으로써, 특히 역가가 높은 코르티코스테로이드를 밀봉요법으로 사용한 경우에 보다 빈번하게 발생될 수 있는 이상반응으로 발생빈도에 따라 나열하면 다음과 같다. : 작열감, 소양증, 자극감, 건조감, 모낭염, 여드름양 발진, 색소저침착, 입주위 피부염, 알러지성 접촉성 피부염, 2차 감염, 피부위축, 선조증, 한진.
1) 이 약을 사용하는 동안에는 약용비누, 연마용 비누 혹은 세정제, 건조작용이 있는 비누나 화장품, 알코올을 다량 함유한 아스트린젠트, 각질용해 약물 혹은 기타 자극물질은 사용하지 말아야 한다.
2) 광과민성을 일으키는 것으로 알려진 약물과의 병용시에는 주의하여야 한다.
Tretinoin에 대한 Mechanism_Of_Action 정보 Tretinoin binds to alpha, beta, and gamma retinoic acid receptors (RARs). RAR-alpha and RAR-beta have been associated with the development of acute promyelocytic leukemia and squamous cell cancers, respectively. RAR-gamma is associated with retinoid effects on mucocutaneous tissues and bone. Although the exact mechanism of action of tretinoin is unknown, current evidence suggests that the effectiveness of tretinoin in acne is due primarily to its ability to modify abnormal follicular keratinization. Comedones form in follicles with an excess of keratinized epithelial cells. Tretinoin promotes detachment of cornified cells and the enhanced shedding of corneocytes from the follicle. By increasing the mitotic activity of follicular epithelia, tretinoin also increases the turnover rate of thin, loosely-adherent corneocytes. Through these actions, the comedo contents are extruded and the formation of the microcomedo, the precursor lesion of acne vulgaris, is reduced. Tretinoin is not a cytolytic agent but instead induces cytodifferentiation and decreased proliferation of APL cells in culture and in vivo. When Tretinoin is given systemically to APL patients, tretinoin treatment produces an initial maturation of the primitive promyelocytes derived from the leukemic clone, followed by a repopulation of the bone marrow and peripheral blood by normal, polyclonal hematopoietic cells in patients achieving complete remission (CR). The exact mechanism of action of tretinoin in APL is unknown.
