1) 녹내장 또는 협우각과 전방이 좁은 경우 등의 안압상승의 소인이 있는 환자(급성 폐색 우각 녹내장의 발작을 일으킬 수 있다.)
2) 이 제품의 성분에 과민증이 있는 환자 |
| 신중투여 |
1) 소아(전신의 부작용이 일어나기 쉽다.)
2) 고혈압 환자
3) 동맥경화증 환자
4) 관부전 또는 심부전 등의 심질환 환자
5) 당뇨병 또는 갑상선 기능항진증 환자

| 이상반응 |
1) 쇽 : 쇽, 아나필락시 양증상이 나타날 수 있으므로 이상이 나타날 경우에는 투여를 중지하고 적절한 처치를 한다.
2) 과민증 : 안검발적, 종창, 안검피부염, 가려움, 발진, 두드러기 등의 증상이 나타날 수 있으므로 이러한 증상이 나타날 경우에는 투여를 중지한다.
3) 눈 : 결막염(결막 충혈과 부종, 눈곱 등), 각막상피장애, 안압 상승 등의 증상이 나타날 수 있다.
4) 소화기 : 갈증, 오심과 구토 등의 증상이 나타날 수 있다.
5) 기타 : 안면홍조, 빈맥, 혈압 상승, 두통 등의 전신 증상이 나타날 수 있으므로 이러한 증상이 나타날 경우에는 투여를 중지한다.

MAO저해제로 치료중인 또는 치료 후 3주가 경과되지 않은 환자와 삼환계 및 사환계 항우울약으로 치료중인 환자에서 급격한 혈압상승을 일으킬 수 있으므로 신중히 투여한다. |

Phenylephrine produces its ophthalmic and systemic actions by acting on alpha 1 adrenergic receptors in the pupillary dilator muscle and the vascular smooth musle, resulting in contraction of the dilator muscle and contraction of the smooth muscle in the arterioles of the conjunctiva and peripheral vasoconstriction. Phenylephrine decreases nasal congestion by acting on alpha 1 adrenergic receptors in the arterioles of the nasal mucosa to produce constriction.

Tropicamide binds to and blocks the receptors in the muscles of the eye (muscarinic receptor M4). Tropicamide acts by blocking the responses of the iris sphincter muscle to the iris and ciliary muscles to cholinergic stimulation, producing dilation of the pupil and paralysis of the ciliary muscle.

Phenylephrine is a powerful vasoconstrictor. It is used as a mydriatic, nasal decongestant, and cardiotonic agent. Phenylephrine is a postsynaptic alpha-receptor stimulant with little effect on the beta receptors of the heart. Parenteral administration of Phenylephrine causes a rise in systolic and diastolic pressures, cardiac output is slightly decreased and peripheral resistance is considerably increased, most vascular beds are constricted; renal, splanchnic, cutaneous, and limb blood flows are reduced but coronary blood flow is increased. Pulmonary vessels are constricted, and pulmonary arterial pressure is raised. This alpha receptor sympathetic agonist is also used locally because its vasoconstrictor and mydriatic action.

Tropicamide belongs to the group of medicines called anti-muscarinics. Tropicamide blocks the receptors in the muscles of the eye (muscarinic receptors). These receptors are involved controlling the pupil size and the shape of the lens. By blocking these receptors, tropicamide produces dilatation of the pupil (mydriasis) and prevents the eye from accommodating for near vision (cycloplegia). Tropicamide is given as eye drops to dilate the pupil and relax the lens so that eye examinations can be carried out thoroughly.
